Series Overview
The Proactive Care Coordination Assistant (PCCA) Program webinar series, presented in partnership with the Physician Learning Program, aims to inform the upcoming PCCA programs across Alberta. The series will demonstrate a standardized PCCA model, provide comprehensive insights into the core groups supported by the PCCAs, and how Electronic Medical Records (EMRs) are utilized for effective implementation.
What is a PCCA?
Proactive Care Coordination Assistants (PCCAs) are essential team members who proactively coordinate care for at-risk groups and arrange preventative health screening within the Patient’s Medical Home. This “behind the scenes” work allows physicians to focus on the patients in front of them, knowing others will not fall through the cracks.
PCCAs at the Edmonton Southside Primary Care Network (ESPCN) help improve continuity of care, preventative health screening rates and panel accuracy for our member physicians receiving this support. They do this by calling at-risk patients and offering annual or screening appointments with their physician, or with the appropriate MDT, as well as confirming patient attachment during each outreach call.
The evident improvement in screening and continuity has led to four PCNs across Alberta adopting this program, and several other PCNs interested. Physicians participating in the PCCA program observe increased panel accuracy, continuity of care, and preventative health screening rates for their patients.
