Proactive Care Coordination Assistant (PCCA) Webinar Series

What is a PCCA?

Proactive Care Coordination Assistants (PCCAs) play a key role in coordinating care for at-risk groups and arranging preventative screenings within the Patient’s Medical Home. This enables physicians to focus on patients while ensuring others don’t fall through the cracks. At the Edmonton Southside Primary Care Network (ESPCN), PCCAs improve continuity of care, screening rates, and panel accuracy by reaching out to at-risk patients, offering appointments, and confirming patient attachment. This success has led to four PCNs in Alberta adopting the program, with more interested. Participating physicians report better panel accuracy, care continuity, and screening rates.
